The information below outlines how your records are protected, how to request access to them, and how to reach out with any concerns.

Notice of Privacy Practices

Your privacy is important to me. As a Licensed Professional Counselor Associate in Texas, I follow federal and state laws (including HIPAA) to protect your personal health information.

This means that:

  • Your information is kept confidential

  • It is only shared with your written consent, except in specific legal or safety situations

  • You have the right to access and request copies of your records

A full Notice of Privacy Practices is provided as part of your intake paperwork and is available upon request at any time.

Requesting Your Records

You have the right to request access to your therapy records.

If your services are provided through a secure client portal or billing platform, record requests may be processed through that system. I will guide you through the correct process as needed.

All requests are handled in accordance with Texas law.

Questions or Complaints

If you have questions about your care or concerns about your privacy, I encourage you to reach out to me directly so we can address them together.

You also have the right to file a complaint with the Texas Behavioral Health Executive Council (BHEC), which oversees licensed counselors in Texas.

Text Messaging & Communication

If you choose to communicate with me via text message or email:

  • These forms of communication may not be fully secure

  • They are typically used for scheduling and brief communication only

  • Therapy content is best discussed during sessions

By choosing to communicate electronically, you acknowledge and accept these risks.

Mobile Messaging Privacy

Your phone number and contact information are used solely for communication related to your care, such as scheduling and appointment reminders.

I do not sell, share, or use your personal information for marketing purposes.

Professional Credentials

Tracy Carlson is a Licensed Professional Counselor Associate in the state of Texas and practices under the supervision of a Licensed Professional Counselor Supervisor, as required by Texas law.
